Work Agreement

All skiing members of the Mad-City Ski Team (or their parents) are required to participate with the following team activities:
  • Participate on a team committee. You will be contacted to sign up for one.
  • Participate in the Clean Lakes Festival (on Saturday, June 16, 2018). If a member is too young, a parent or designated individual may work for them. Any member under the age of 18 may have their parents substitute for them in all work requirements.
Each skiing individual or the head of the household will be asked to sign an agreement stating their understanding of the necessity of these work days, their intent to participate and understand that there are ways to earn back dues paid through fund raising opportunities. The skiing member listed below understand that it takes thousands of hours to put together a successful ski show. I/we also understand that in order to be a successful team, it is imperative that every member help share the workload. By signing this agreement, I/we are making the commitment to participate on a committee and work at the Clean Lakes Festival.

Athletic Waiver

In consideration of being allowed to participate in any way in Mad-City Ski Team programs and related events and activities, the undersigned:
  1. Agree that prior to participating, they each will inspect the facilities and equipment to be used; and if they believe anything is unsafe, they will immediately advise their coach or team leader of such conditions and refuse to participate.
  2. Acknowledge and fully understand that each participant will be engaging in activities that involve risk or serious injury, including permanent disability and death, and severe social and economic losses which might result not only from their own actions, inactions or negligence, but the actions, inactions or negligence of others, the rules of play or the condition of the premises or of any equipment used. Further, that there may be other risks not known to use or not reasonably foreseeable at this time.
  3. Assume all the foregoing risks and accept responsibility for the damages following such injury, permanent disability or death.
  4. Release, waive, discharge and covenant not to sue Mad-City Ski Team, its affiliated clubs, their respective administrators, directors, agents, coaches, and other employees of the organization, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event, all of which are hereinafter referred to as “releases”, from any demands, losses or damages on account of injury, including death or damage to property, caused or alleged to be caused in whole or in part by negligence of the releases or otherwise.
All memberships will expire on December 31, 2016. All members are required to be affiliated with USA Water Ski (USA-WS), and register for insurance purposes register with an active USA-WS membership. All members shall abide by all rules and regulations of the Mad-City Ski Team, as set forth in the Mad-City Ski Team bylaws. THE UNDERSIGNED HAS READ THE ABOVE WAIVER AND RELEASE, AND UNDERSTAND THAT THEY HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT AND SIGN IT VOLUNTARILY. I HAVE READ AND UNDERSTAND THE RULES AND REGULATIONS AS SET FORTH IN THE MAD-CITY SKI TEAM BYLAWS.

Photo Release

In consideration of being allowed to participate in any way in Mad-City Ski Team programs and related events and activities, the undersigned:
  1. The ski team is authorized to use or cause to be used still photographs or video footage for advertising and publicity of the ski team. The photos may be used individually or in conjunction with other photographs.
  2. The ski team is authorized to reproduce and use, or cause to be reproduced and used, such photographs. I understand others may use and/or reproduce the photos with or without Mad-City's consent.
I hereby release the Mad-City Ski Team and it appointed officers from all claims of any kind on account of such use.
